Overcoming a Rotator Cuff Tear Without Surgery: A Real Success Story

The Patient’s Journey

Two months after a road traffic accident, this patient came to our clinic with significant shoulder pain and dysfunction. Despite being denied an MRI, he was diagnosed through an X-ray with a rotator cuff tear.

On evaluation, he showed:

✅ Atrophy of the infraspinatus
✅ Anterior humeral instability
✅ Long head of the biceps tendon irritation
✅ AC joint pain
✅ Restriction of shoulder flexion, abduction, external rotation, and horizontal adduction

He tested positive on Speed’s test, Yergason’s test, Hawkins-Kennedy, and Neer’s impingement tests. Neurologically, there were no weaknesses or altered sensations, but ULNT1 testing revealed median nerve mechanosensitivity.

Clinical Approach

The big question: WHERE TO START?

Our initial focus was manual therapy to improve range of motion and stabilize the anterior shoulder. This worked well, but the infraspinatus atrophy was still a major obstacle.

Many traditional resources (social media, videos) recommend only external rotation exercises at 0 degrees of flexion and abduction. In my clinical opinion, this is too restrictive and ineffective for restoring true rotator cuff function through the entire movement range.

Instead, we designed an exercise program activating the posterior rotator cuff during shoulder flexion and abduction, while simultaneously addressing irritation of the long head of the biceps.

A look back at the anatomy revealed how limited median nerve mobility could contribute to ongoing irritation in a crowded shoulder space.
